Critical Illness
The voluntary Critical Illness coverage provides a lump sum benefit in the event of a diagnosis of a covered illness and is insured by Prudential. Employees may elect coverage for yourself, your spouse, and your child(ren) with no health questions. Covered conditions include cancer, heart attack, stroke, major organ transplant, coronary artery bypass surgery, end stage renal failure, and more.
Benefits options include:
Employees: From $5,000 to $30,000 in increments of $5,000
Spouses: 50% of the employee amount (Employees must be enrolled to elect spouse coverage.)
Children: 50% of employee coverage for all children (Employees must be enrolled to elect child coverage.)
Features
- Benefits are paid in addition to any other insurance that you may have, and benefits are paid directly to you
- This plan pays a benefit if the initial diagnosis for a covered illness is while the certificate is in force.
- This product will pay multiple times for the same or different covered conditions
- Benefits may be used however you'd like. Typical uses include:
- Out-of-pocket medical and non-medical expenses and home health care needs and home modifications
- Recovery and rehabilitation
- Child care or caregiver expenses and travel expenses to and from treatment centers
Includes a wellness benefit for covered preventive screenings in the amount of $50 per covered member per year.
How to File a Claim Electronically:
- Visit www.prudential.com/mybenefits and login or register to complete your form electronically.
- Select “My claims” from the left side menu and click on the option to “File a claim/Report an absence.”
- Provide authorization for Prudential to obtain necessary medical information from your doctor, saving you time.
